We kindly ask that new patients fill out their health history forms to us ahead of time, if possible.  These are located under the “New Patient Forms” link.  (Fertility patients, please fill out the fertility form as well).  Important: all insurance patients please verify insurance coverage prior to your initial visit.

 

EXCHANGE

Please call our office for current rates.  Rates may vary depending on the service(s) provided.

Payment or co-payment/ co-insurance is due at the time of service.  Self-pay rates include a prompt payment discount.  For your convenience we accept cash (exact change appreciated), check, Venmo, Visa, MasterCard, Discover and American Express.  Flexible Spending Accounts (FSA) and Health Savings Accounts (HSA) with a major credit card logo are also accepted.

 

INSURANCE

We currently accept the following health insurances: Anthem Blue Cross/Blue Shield and Harvard Pilgrim. Please note that unfortunately, we are unable to accept Medicare at this time. Prior to your initial visit, please verify that your plan has acupuncture coverage, any deductible, co-pay or co-insurance, number visits allotted per year, and any other specifics or restrictions on your policy.  If your plan does not cover acupuncture, or if we are not in network with your insurance provider, you will be responsible for full payment at the time of service, less a time of service discount.  Where reimbursement is possible, we will be more than happy to supply you with a superbill.
