If you are making an initial appointment, please print out the New Patient forms at nhacupuncture.com, fill them out, and bring them with you to your first appointment. Please also include a copy of your insurance card, if applicable. If you are unable to print out your forms, please arrive 15 minutes early to fill them out here.
All patients who plan to pay with insurance, are asked to please verify coverage (including any restrictions, limitations, deductibles, co-pays, co-insurance, and number of visits allotted) prior to your first visit. You are responsible for any visits not covered by your insurance.

We are located on the 1st floor of the Governor's House, in the Nubanusit Neighborhood and Farm Community. Once you enter the neighborhood, the Governor's House is the 1st building on the right. There is a large maple tree in front of it. Please park in a Visitor's Parking spot.

COVID policies and procedures:
1. When you arrive, we may screen you to confirm the absence of COVID symptoms or exposure.
2. We will not be treating anyone in person with COVID. Patients will be advised to self-isolate and call their PCP. We are available to treat COVID symptoms with Chinese herbal medicine via telehealth.
3. All patients are required to sign a COVID consent to treat form.
4. Masks are requested but not required at this time.
5. We follow the state of NH's safe travel guidelines.
